Ensure your health care wishes will be fulfilled, even if you can't speak for yourself. Learn how to communicate your values and goals about medical treatment options and document your choices using Advance Healthcare Directive and POLST forms. We’ll also discuss how to choose the right person to make decisions for you if necessary.
